AI-assisted documentation

Separate permission for an explained documentation workflow

Effective October 24, 2026

With your permission, the practice may use an approved AI documentation tool to help prepare the clinical note. The clinician remains responsible for reviewing, correcting, and signing the note. AI does not independently diagnose you, choose treatment, or replace the clinician’s judgment.

Your choice

You may decline AI-assisted documentation or ask that it stop for future use. The clinician will document the visit manually if you decline. Your decision does not prevent you from receiving otherwise appropriate care. Permission can be revisited at any time.

Information handling

Before use, the practice will explain the selected tool, the information it processes, and how that information is handled. The practice will select and use only a service, contract, and configuration that meet its requirement not to retain audio or video recordings of your visit. This standard does not mean that no sound is processed during the visit; a tool may process live audio to draft the note. The reviewed clinical note is retained as part of your medical record. Any transcript, temporary processing, storage, deletion, access, or secondary-use details will be explained for the actual tool before consent is obtained.

Before the tool is activated

The practice must verify its chosen arrangement, including any required privacy agreement and the vendor’s handling of information. If the arrangement does not meet the no-retained-recording requirement, the practice will not activate that workflow. This consent does not authorize unrelated recording, marketing, sale of information, or sharing beyond applicable privacy permissions.
